Features Passport for Care
The Passport for Care is a free online resource tool for survivors of childhood cancer (including patients who received a bone marrow transplant).
The Passport for Care allows survivors to easily access their treatment summary and long-term follow-up care recommendations.
To access the Passport for Care, survivors enter a code provided by their treatment clinic.
If a code is not available, a survivor or provider can anonymously access follow-up care recommendations based on the treatment the survivor received (chemotherapy, radiation, or certain types of surgery).
The long-term follow-up care resources can be printed or saved by the survivor and shared with their health care providers.The long-term follow-up care recommendations in the Passport for Care are based on the Children’s Oncology Group “Long-Term Follow-Up Guidelines for Survivors of Childhood, Adolescent, and Young Adult Cancers.”
